The District II Committee on Children with Disabilities has developed a standard health form for sharing information on special needs children form abstracted from the AAP “Emergency Form for Children with Special Needs” and the Medical Home Improvement Project “Care Plan.” You can download the form by clicking HERE and viewing the document in Microsoft Word.
